- Enrolled in a NYS Health Home (Adults and Children)
- Individuals with Substance Use Disorder (SUD)
- Individuals with Serious Mental Illness (SMI)
- Individuals with Intellectual and Developmental Disability (IDD) (Adults and Children)
- Pregnant and Postpartum Persons
- Post-Release Criminal Justice-Involved Population with Chronic Conditions, SUD, or Chronic Hepatitis-C
- High Risk Children Under the Age of 18
